有个身手不凡的无名小偷，在狱中得到老郎中赠送的医书后改邪归正，开始悬壶济世，还自称郎中。有一天，郎中救了郭家庄一名受伤的家丁，却不知道这个家丁是因为郭家行事霸道，被乡民追打才受的伤。不久后，这名家丁遭人暗杀，尸体手中握着“三石”。郎中碰到另一家丁企图奸污村女陈小圆，便出手相救。郭庄主的女儿佩青以为郎中是杀害家丁的凶手，把他吊在了树上。小圆救了郎中，还帮他开了医馆。郎中医名越来越大，佩青召他为庄主治病，可庄主服药后却中毒身亡，佩青便把郎中囚禁在地牢。庄主的养子磊救出了郎中，还指出佩青是下毒之人；郎中差点误杀了佩青，又被磊用毒镖打伤。郎中识破了“三石”的隐喻，原来是磊想谋篡庄主之位，于是他把真相告诉了佩青。郎中与磊展开激战，小圆被杀，郎中愤怒地斩杀磊后，从此浪迹天涯。
