《真实恐怖故事》改编自2015年获奖同名热门Podcast节目，由 Aaron Mahnke 编剧，透过检视真时历史事件探索人类的黑暗本质。打着「有时真相比虚构的故事更可怕」的口号，节目中所有故事源於真实调查丶目击者报告和历史档案。从吸血鬼传说丶活尸的起源丶狼人到美国史上第一个连环杀手 H.H. Holmes 皆为《真实恐怖故事》触及的题材。

投稿された実話を基に手掛けられた５話から構成されるオムニバス・ホラー。いつ誰の身にも起こりうる身近な恐怖をリアリスティックに描く。監督はこれまでオリジナルビデオの分野でホラー作品に携わり、本作で劇場映画初メガホンの山本清史。出演は「仮面ライダー５５５」の半田健人、元パイレーツの西本はるか、グラビアアイドル仲根かすみ、ほか。 　　「深夜の警備員」――警備員・神野は巡回中のビルのトイレで、目の周りが窪んだ青白い唇の男たちに遭遇してしまう…。「首坂」――沙知恵は線路沿いの夜道で、独りの白い服を着た女を見かけるが…。「葬儀屋が見たもの」――室田が葬儀の準備に訪れた高橋家は、赤い服を着た女の霊に取り憑かれていた…。「鼓膜移植」――ドナーの少年のおかげで手術により、事故以来耳が聞こえるようになる理恵だったが…。「エレベーターの少女」――恵美はエレベーターの前で奇妙な少女と出会うが、エレベーターは少女を乗せずに上昇していく…。

Five carpoolers travel in a motorhome to reach a common destination. Night falls, and to avoid a dead animal carcass, they crash into a tree. When they come to their senses, they find themselves in the middle of nowhere. The road they were traveling on has disappeared and there is only a dense, impenetrable forest and a wooden house in the middle of a clearing, which they discover is the home of a spine-chilling cult.
