As the unsuspecting young women, Erika and Amy, get in their car, a sadistic predator in black equipped with a gas-mask knocks them out and abducts them. From this point on, an endless nightmare of unspeakable torment awaits the helpless victims--who are drugged and chained down by their throats to wooden tables--through a wide assortment of sharp instruments and common household tools. Always starting from left to right, the butchering begins, as a savage trio of desensitized executioners film the abominable act of sacrifice with their Super-8 camera. Is death the only means of salvation?

Haunted by the death of his father and other psychological traumas, Daniel (Roberto Scorza) returns to the home where he was raised. Faced with intense emotional scars, as well as physical -- which are realized by the years of self-harm depicted by the cuttings adorning his body -- he enters the bathroom to begin a journey of self-exploration, self-mutilation and quite possibly, self-enlightenment. Prepared only with three white candles and some crude instruments, Daniel attempts to beckon the embrace of the Goddess Ishtar (Flora Giannattasio) to assist him on his self-illumination. Beginning with a blade, Daniel digs into the palm of his hand, as deep crimson flows into the bathtub. Lapping up the blood from his fresh gash he sexually caresses the wound with his tongue (ala cunnilingus), thus commencing his voyage.

Trapped in the padded white room of an abandoned asylum, a helpless man awakens, only to find himself chained down to a medical chair. There, a demented surgeon and his equally deranged assistant embark on a seemingly endless series of chirurgical tortures, unlocking new levels of pain and suffering, one excruciating procedure after another. Intent on harvesting the captive's endorphin-infused blood, the maniacal doctor mutilates the feeble flesh and breaks the naked bones, until the phial is filled with the life-giving substance. However, in this hellish purgatory of anguish and torment, the man finds the strength to cling to hope, as he keeps finding hand-written notes in his grim cell. Could there be another tormented patient in this nightmarish facility? Is there an escape from the never-ending agony?
